 ellness visits to the community at Northern Lake YMCA\, 2000 Western Ave
 .\, Waukegan\, on Thursday\, July 12\, and Wednesday\, July 25\, from 7 
 to 9:30 a.m. both days. These exams are available at no cost to those wi
 th Medicare or commercial insurance. There is no co-pay or deductible. M
 edicare recipients need to have been in Medicare at least one year and h
 ave not had an annual wellness visit within the last 12 months.\nPartici
 pants receive a personal report that outlines their current health and r
 isk status and formulates a personal prevention plan. The exams also are
  ideal for people who don&rsquo\;t have a doctor or haven&rsquo\;t seen
